Description
Used in the investigation of primary and secondary aldosteronism and in the treatment of hypertension.
Test Preparation Instructions: For upright measurement, the patient must be ambulatory for 30 minutes prior to collection. For supine measurement, the patient must be lying down for at least 30 minutes prior to collection. Renin concentrations are impacted by the following: salt intake; age; gender, menstrual phase, pregnancy and use of oral contraceptives; time of day; posture; chronic kidney disease; and use of diuretics, dihydropyridine calcium channel blockers, ACE inhibitors, ARBs, beta-blockers, clonidine, alpha-methyldopa, NSAIDs, and direct renin inhibitors. When possible, these medications should be ceased for at least two weeks, ideally 4-6 weeks, prior to collection. During this period, antihypertensive medications with minimal effects on renin may be used in order to maintain control of hypertension (e.g., verapamil slow release added to hydralazine and/or alpha-blockers).
